Living with and beyond cancer
Patient and Public Resources
Life after a cancer diagnosis is about more than treatment β it’s about living well and feeling supported every step of the way. This section provides trusted resources to help you manage the physical, emotional, and practical challenges of life during and after cancer.
You’ll find guidance on staying active and healthy, coping with fatigue and sleep issues, and accessing support for work, relationships, and mental wellbeing. We also help you to understand personalised care approaches, including Holistic Needs Assessments, End of Treatment Summaries, and Personalised Care Planning, so your care reflects what matters most to you.
We aim to give you and those close to you the knowledge and support to live well, with care designed around your needs and resources to help you flourish beyond cancer.
You will also find links to local support services on our patient support page.
Personalised Care
We want to make sure everyone diagnosed with cancer feels involved in their care – we call this personalised care. This short video explains what personalised care is, what to expect following a cancer diagnosis, and how it can help you live as full and active life as possible. This includes a Personalised Care Support Plan and End of Treatment Summary.
The next short animation explains the benefits of completing a Health Needs Assessment (HNA). It supports patients to think about key areas in their life and highlights if they have any particular worries or concerns. This assessment helps patients receive support and advice that is tailored to their individual needs when they need it most.
